Co-payments apply to certain services. Our HealtheDividends program uses economic incentives to encourage those enrolled in the medical plan to establish a relationship with a primary care physician and complete a yearly Health Risk Assessment.
The dental plan covers services at 100%, 80% or 50% up to the plan's allowance when the dentist is in-network. The maximum amount paid for covered services is $1200 per person per calendar year.
